Improved access for patients, with the development of hubs providing weekend access.
This is being operated out of Ashingdon Medical Centre and Essex Way surgery on a Saturday and Sunday. Please contact your local surgery to book appointments at either of these sites.
We have been commissioned by the CCG to do the mop up on outstanding learning disability health checks. This is to ensure this vulnerable group of patients health needs are looked after.
GPHA has been an integral partner in developing this service, providing the leadership to ensure the service works effectively for our patients and practices. The care co-ordination teams will be working with practices to identify high-risk frail and elderly patients with the aim of managing them proactively. Their work will also support and build on the unplanned admissions enhanced service.
GPHA will also be providing clinical leadership to the service.
On 17 June 2014 GP Healthcare Alliance officially formed and began trading. The organisation is now in the early stages of developing pilots for the area.
